A: You will receive a phone call from a staff member of the Miners Hospital to schedule your appointment.
Q: I don't have documents that support my claim that my injury or illness is related to working in the mining industry. Does that mean I am not eligible for services?
A: No. Documentation is extremely helpful, but is not the only way to determine eligibility. We will also ask you for your work and health history.

A: Any other insurance coverage you have will be billed first. If you don't have insurance, eligible services will be billed to the Miners Trust Health Plan.

A: Call the Miners Hospital toll-free number at 866-864-6377. We will ask you to fax or mail the bill to us so we can resolve the
problem.

A: You can get medications from one of Ï㽶ÊÓƵ of Utah Health's pharmacies. But, the Miners Trust Health Plan will only cover medications directly related to a mining injury or illness. Prescriptions must be written by a Miners Hospital doctor.
Q: If I receive services through the Miners Hospital and Clinic, will I continue to see my family doctor or will I be
expected to transfer all my medical care to the Miners Clinic?
A: Miners Hospital doctors will see you for mining-related problems only. You will continue to see your local doctor for all your other health care needs.
