UUHC named an Anticoagulation Center of Excellence

The has been named an Anticoagulation Center of Excellence after passing the Anticoagulation Forum's assessment program.

"This designation recognizes our extraordinary commitment to the highest quality of care," said Chief Pharmacy Officer Linda Tyler, PharmD. "For our patients, this means that we will not only ensure that each individual patient is on the best anticoagulation medications for him or her, but we will support our patients as they move through our system to receive care from other providers. This coordination of care is critical for patients on anticoagulation therapy."

To earn this recognition, centers are evaluated on drug therapy management, disease state management, transition and coordination care, service operational performance and patient and family education.

The Anticoagulation Centers of Excellence program was created in 2012 to help health care professionals provide the highest level of care and achieve the best possible outcomes for their patients on antithrombotic medications.
