Referral Information
Most medical specialists will accept only referred patients. This is mainly to try to ensure that the specialist you are seeing is appropriate for you and your condition.
Check with your insurance company to see if a referral is necessary.
Cancel an Appointment
To cancel an appointment phone the office during business hours. You must allow at least one days’ notice so that we can offer your appointment time to patients on our waiting list.
We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.

First Visit Guide
What to Bring to Your First Visit
For your first visit you will need to bring a referral letter from your physician if necessary. Here is a list of other items you should bring:
• Driver’s License or a valid ID
• Insurance information
• Referral letter (if required)
• Reports, X-rays, MRIs, CT scans, and any other relevant information
• List of medications (if any)
We encourage you to bring a written list of questions. This will ensure you don’t forget to ask them when you are seeing the doctor.
Medical Record Confidentiality
Your medical file is handled with the utmost respect for your privacy. Our staff is bound by strict confidentiality requirements as a condition of employment regarding your medical records. We will not release the contents of your medical file without your consent.

COMPLIMENTARY REVIEW OF X-RAYS
To determine if an office appointment is beneficial for you, we provide a complimentary review of your hip X-ray, MRI, or CT scan.
